I dont do very many GYN surgerys so i need some help please.

Pt had Laparoscopic exam, with dissection of adheasions of the bowel - pelvic sidewall along with Right Oophorectomy.

I found 58661 for the Oophorectomy. But i'm stumped on the Bowel/pelvic Adhesions and how to code it.

I have been told that if dissecting the adhesions was part of releasing the ovary so it could be removed or to clear the surgical field to enable access to the ovary, it is considered part of the Oophorectomy and would not be billed separately.

Lysis of adhesions is bundled with just about every procedure as an integral component (aka "separate procedure"). If the adhesions are dense enough and required an above average amount of time to remove, you can use mod -22 on the primary procedure BUT the documentation MUST support the extra work (increased procedural service).
