My question is: I have a coder that I am training and in the A/P the provider indicates dx. as H/O hernia and gerd, from reviewing the notes the patient is on meds for these diagnosis and also in the A/P the provider indicates the meds he uses for the hernia and gerd, would you code the hernia (550.90) and the gerd(530.81)? Thanks.
