Physician is billing an OV on date of surgery. Ex: 99215-25, 46221 POS 22
Initial decision for surgery was made at previous encounter. The physician is billing a level 5 E/M code for H& P. To my understanding H&P s are included in the surgical package. The physician stated that she has been billing this way for seven years and insurance pays the e/ m code. Do different rules apply when these services are billed in the outpatient setting?
