He charged a subsuquent visit.
 
This sounds like a case where perhaps one of your physicians was covering the other's patient.

I assume the patient was already in the hospital as an inpatient and one of your physician's saw the patient for the H&P and the second physician saw the patient after, charged a subsequent visit pre-op and then performed surgery. Is that the case? If that is the correct scenario then it depends on what the documentation contains and when the decision for surgery was made.
