There is a program called H2H -hospital to home.

I believe it is mandated by Medicare to prevent readmissions to the hospital after a dx of CHF.
Is anyone else doing this and know the guidelines for billing?
 
I haven't heard of this but I'm with you; I need information on the guidelines. We have a lot of CHF Medicare pt's and I imagine this might affect us.

I'll what I can find out from our rep.
