I got back a medicare denial for cpt code 11042 am currently asking for forum or guidance on the oper note
PREOPERATIVE DIAGNOSIS: Left hand wound.
POSTOPERATIVE DIAGNOSIS: Left hand wound.
PROCEDURE PERFORMED: Left hand wound debridement and application of Clarix.
DESCRIPTION OF PROCEDURE IN DETAIL: After appropriate consent was done, the patient was placed in the lateral decubitus position, the area was prepped and draped in an appropriate fashion. Using a scalpel, debridement was performed over the skin and subcutaneous tissue until all the devitalized tissue was removed. The wound measured 30 x 35 mm. Application of Clarix 2 mL and plasma was performed and then covered with sterile dressing. The patient tolerated the procedure well.
ESTIMATED BLOOD LOSS: Minimal.
