Wiki Having trouble getting paid for TB test in pediatric practice

I work in pediatric practice and I am having issues to get TB test paid
When Nurse do TB testing I code
99211 with 25 mod and
86580 with dx for encounter and if they need for school, education or job purpose
And most of Ins company rejecting saying mod missing or not paying 99211 and 86580 both
Could you give suggestion we have lot of 17 and above kids need TB test either for education purpose or for Internship and employment
Please help

We bill only the TB test 86580 on the date of service. You should not report and E&M code unless there is some other E&M component happening at that visit. You may bill the 99211 when the patient returns to have the test read. However, due to the nature of the quick check, a note to complete (all done by the nurses), we do not charge for the patient to have the TB test checked.
