trying to find directions (CCI?LCD?) regarding billing same day visits for HBO (Hyperbaric chamber tx) and other procedures ie: E/M or Debridement. I believe this is ok, as long as mod-59 is applied (or-25 to E/M) as the reason for the additional visit is distinct different visit. HELP
