Does anyone know where I can find the list of HCC codes - Pacific source medicare states there are 79?? When I look on Medicare's website, I find a huge list but am still sure what this really means. Do you also know if the codes change per year?

HCC codes

Hi! There are 79 categories with over 8,000 codes included in the categories. They do change every year, just like ICD-10 does, with more added or adjusted. The AAPC ICD-10 book does mark the HCC codes for you.
