If patient has a chronic condition noted in HPI, PMH or Assessment/Plan can the dx be captured without meat/tamper? I am getting conflicting information. Also does anyone happen to have a concurrent chronic condition list they would share that is more current that originally posted by CMS?
 
Yes you can. If the provider said something like patient is here today to establish. Patient has chronic heart failure with chronic kidney disease stage three then it would be appropriate to add into the claim. Also, if a condition is in the past medical history and they have medication to support one on one diagnosis you can capture that as well. For example, if the patient had insulin in their medication‘s and diabetes in the past medical history, it would be appropriate to pick up the diabetes codes.
