Is anyone aware or know if its true that there is a list from Medicare of HCCs that do not need an evaluated statement in patient chart note? or is this false? all conditions need an evaluated statement
 
I have never heard of an HCC that didn't require an evaluative statement.

I don't know which ICD-10 book you have, but I have the Expert for Physicians edition (through Optum) and there is a section that talks about HCC coding - Appendix E.
