I took the webinar on improving risk adjustments and it talks about how the provider receives a higher payment per month/year if the patient has multiple HCC conditions. However, it didn't go over where you would see these payments. Are they a part of the MIPS incentive? Would we receive it on each claim? Because Medicare has flat rates, so I'm just wondering where/how I would be able to show the providers the results if they were to put more effort into HCC. Also wondering if it's the same between straight Medicare, and Medicare Advantage plans. Thank you!
