Does anyone know when you would use the HCPCS annual exam code S0612 instead of the 99394-99397? Thanks for you help.
 
I guess what I really need to know is if anyone uses these on a regular basis for Medicaid patients and what are the requirements for doing so. Do you always use the -FP modifier or only if this is to start or continue birth control?
