We are debating whether the HCPCS codes go by dosage or NDC number. For example,
J1070 is up to 100mg of testosterone cypionate, J1080 is 1cc 200mg testosterone cypionate. We only order vials of 200mg/cc. If a patient is given .5cc, 100mg, should we use J1070 1 unit or J1080 .5 unit?
