When is it correct to bill the Q0091 cpt with 99385-99397 annual preventative exam?
Is it ever appropriate to link z12.4 with Q0091 when it is done in a routine annual exam?

Patient is NOT medicare.

As per the ICD CM z12.4 falls in the exclude1 category for annual preventative examination.
I am following the guidelines so I am trying to figure if something has changed that I am not aware of.
