Specific to Medicare: When is it appropriate to use a V2499? Is V2499 always progressive? I know Medicare does not pay this hcpc code. Can you bill for a V2781 w/correct split out of lens even though the patient orders a V2499?
Can a V2430 be a traditional bifocal or a progressive? If you are locating this information online can you please include a link in your response?
