Any input on mandatory health insurance physical billing. We were kind of leaning toward the preventitive codes but also were questioning 99450. Any help with how to bill these correctly would be appreciated.
 
Hello, I would say it is specific to the payer.
I know for BCN in Michigan, their "Healthy Blue" visit pays $40 and will bill out the 99080 form fee code.
HealthPlus in Michigan only pays $30 for their "Healthy Solutions" visit with the CPT 99401.

I would contact the payer and ask them what they want!
