in need of a dx code for a healthy adolescent that comes in to discuss diet, & use of supplements for strenght building such as creatin. no V codes please.
 
There is no other choice. This is a V code for counseling or just well check. You cannot assign a dx to a patient that is healthy with no complaints. Why though would you not want a V code?
 
Payment is not the entire issue. You have hit on a coverage issue. You cannot "give" the patient a dx where one does not exist. The payer is allowed to establish payment/coverage parameters, but that does not allow us to submit a dx code for any other that what is documented in the medical record. Sorry but what you have is a V code there is nothing else there. Remember the dx is the patients not the doctors or the payers. If this were your son or daughter you would want the dx to be exactly correct.
