What are the recommendations for documenting how the HPI is obtained with a hearing impaired patient?

How is it obtained?

If they have a interpreter then you would document that and it would also give you an element of social history as well as past history. So the documentation might say "Patient presents today with interpreter due to being hearing impaired, patient is complaining of...." and just continue on with the normal HPI.

If they are writing things down I would think it would be documeneted the same way you just wouldn't automatically get the social history piece.

This is just my opinion and I can't ever remember seeing this for hearing impaired but I did see it a lot for people that did not speak english.
