I have a question on modifiers for: a patient has had a heart transplant by a CV surgeon, but the cardiologist in the group is also seeing the patient for unrelated transplant issues and not post-op, what modifier can be used?

help!!!!
 
Modifier 24 - unrelated evaluation and management service by the same physician or other qualified helath care professional during a postoperative period
