Wiki HELP! UHC denials of DME

I'm wondering if anyone else is having this issue and if so can you please give me some advice. A shoulder immobilizer (L3960) was dispensed to a patient following outpatient shoulder surgery right before discharge. The claim was submitted as follows:

29827-LT with place of service 22
29826-LT with place of service 22
L3960-NU,LT with place of service 12

The following supporting documentation was sent with the original claim:

Operative Report
DME Orders
DME Delivery Ticket - signed by the patient on the day of surgery

UHC denied L3960 because it was signed for at the hospital and the documentation cannot prove that the patient is using the brace at home. Has anyone had experience with this kind of denial? I always bill DME with POS 12. UHC says they are following CMS guidelines but I can't find anything that directly addresses that the DME delivery ticket needs to list that the device will be used at home.
