Our physician did a TCM visit 99495 along with cryo 17000. We add the modifier 25 to 99495 but it is being rejected out of our clearing house as;
99495 - SMARTEDIT MUN PER MEDICARE CCI GUIDELINES PROCEDURE CODE 99495 HAS AN UNBUNDLE RELATIONSHIP WITH HISTORY PROCEDURE CODE 17000 BILLED FOR THE SAME DATE OF SERVICE.
Has anyone had any problems with this? Can they not be billed together?
For reference we are billing RR Medicare.
