I have a question on how to bill a labor epidural for the scenario below.

MD performs pre-op visit
MD prescribes labor epidural
MD personally places the epidural
MD available from 8:30 am – 3:00 pm
CRNA removes epidural after delivery at 8:30 pm
CRNA performs post op visit same day
MD performs post op visit next day

Would the claim be billed under the MD or CRNA?
I want to say that the claim should be billed under the MD since this was a vaginal delivery, not requiring surgery, and because of the change in the Interpretive Guidelines that classifies labor epidurals as analgesia versus anesthesia. The physician performed the critical elements of the procedure (pre-op, anesthesia plan, personally inserted epidural, and post-op the following day) BUT the physician left a few hours before the epidural was removed so I am questioning how this should be billed.
