Wiki Help billing Wound care supplies

Is there anyone that bills wound care supplies that actually gets them paid? If so, can you please help. We are billing the below codes to Molina Marketplace but it doesn't matter where we bill them (except Medicare) we can't get them paid.
A4450 AW A1
A6402 A1
A6446 A1
A6196 A1

The A6446 did pay and A4450 was denied as non covered and left to patient. Does anyone have a secret or insight on how to get these covered? I would be so grateful. thanks so much.

What is the POS? Facility or profee?

We are billing POS 12 because they are being mailed to the patient's home. They are seen in the office, the order is placed and then a supply company ships them out.
