Wiki Help code 29891 vs 29892

Please help shed some light. What is the differnece between codes 29891 and 29892? They read pretty much the same except 29892 says "large"
lesion.

I am having a hard time trying to determine which one is the proper one to use. In various op reports there is never mention of the OCD lesion size.
