Patient underwent Exploratory laparotomy, small bowel resection, sigmoid resection, placement of ABThera. DX K56.609 small bowel obstruction. Surgery was cut short due to patient becoming hypothensive and requiring pressor support. That is why the ABThera . I am coding 44120, 44140, would like to add 97606 for the ABThera, but I don't know what to use as a Dx. code.
Two days later he was taken back for surgery, ileocolic resection w/anastomosis, low anterior resection, abdominal washout, and the ABThera. Dr. Wants 44145 to be primary code. I will need modifier 78 or 79 - I am thinking 78 because they kind of knew they were going back in. Pre operative Diagnosis - Status post small bowel resection complicated by sepsis. DX?? T81.9XXA or use the original dx of small bowel obstruction?
Next day back to OR - 3rd look laparotomy, abdominal closure with bridging mesh, end colostomy creation. Dr. Says Ventral hernia repair w/colostomy. I was just going to do 44320 for the colostomy. Again Modifier 78 I would think. Again - what dx do I use - the original obstruction diagnoses. Post procedural complication code?
Any suggestions? If you can point me to the correct coding sources so I can read them too.
