Wiki Help! Coding question

My providers are very good at dictating elevated lab results, i.e. creatinine, white count, other various abnormalities. Do I code this abnormality out? Or do I ignore it unless they specifically state that the pt has, thrombocytopenia, or whatever? I would love input on this.
