Hello all,
Please help me with the following:

cc: left elbow pain
the patient failed nonoperative treatment for osteoarthritis of the knee
pt here for 1st synvisc injection ,16 units

plan: f/u 1 week for her 2nd synvisc

the patient was seen and examined in the absence of the doctor

I coded J7325 x16, 20610-LT with DX M17.12 Is an E@M needed?
 
If the patient was seen in the absence of the physician, then who administered the injection. Also the type of osteoarthritis is not documented so you would need the dx code M17.9 .
