Rib protrusion is same as rib dislocation and this will be coded with Dx. 839.69 - Closed dislocation other location (Rib).

I would not presume a dislocation if that's not specifically documented.
Could it be Pectus Carinatum? Or is the patient so thin his/her ribs protrude?
Need more info to give you an accurate response.
