Patient came in to follow-up on her dizziness. Since the patient no longer had dizziness, the physician wrote 'hx of dizziness' as the diagnosis. How should I code hx of dizziness? Thanks for the help!! :)
 
If you can not find anything to reflect "hx of" you can code the dizziness. When a patient returns for follow up on a recent dx and it has improved, you can still code the condition that brought him/her in
 
If the patient came in specifically for follow-up of her now resolved dizziness, I would code it to a follow-up visit V67.59.
