Wiki Help!! I can't find a code!!

My doctor has wrote letters, for 2 of his patients , for admission to a nursing homes. He did review these patients charts. He spent around 1/2 hour on each. He is asking me how he can bill Medicare for this. I looked at the Care Plan Oversight codes, and I didn't feel they were quite what I was looking for. If anyone out there knows how I can code this I would be grateful for the information.
 
Without a face-to-face service, you cannot bill Medicare--typically the primary care physician will do the Nursing Home admission, and bill out that service when he sees the patient face-to-face for that initial assessment. But a letter (without an in-person visit) doesn't meet the criteria.
