How do I code for this L1 fx?
"Posterolateral fusion with pedicale screws of T11; T12; L1; and L2 bilaterally using morselized allograft using BMP"

I'm thinking it should be 22325, 22612, 22614 x3. My doc is saying 22614 should be x4.

I'm thinking that 22325 covers the L1 and 22164 covers T11; T12 and L2.

Any thoughts?
