Can we code Z45.2 or Z51.11, If patient coming for central venous access needed for chemotheraphy, Patient having breast cancer
 
Where I've worked, we could code the breast cancer as the primary diagnosis for the encounter for placement of the access device since that procedure is a part of the course of treatment for the disease. As I understand it, Z45.2 would be used for an encounter at which the access device is being adjusted or removed at the end of the course of treatment, and not for the original placement. Z51.11, per the guidelines, is used for an encounter which is 'solely for the administration of chemotherapy', and would be followed by the code for the breast cancer as a secondary diagnosis. This code would not be used for the encounter to place the device.
