someone can help me out why unitedhealthcare denied cpt 92015 after submitted with modifier "GY"
 
The GY modifier indicates that the service is statutorily excluded and expected to be denied, but no ABN has been signed.
OR
it is denied for invalid modifier, if United does not recognized HCPC II modifiers, which are typically used on Medicare claims only.
