Giving that I audit and find that the documentation supports the additional office visit, would this be the way to code this. I tell them all the time do not do this and they do not listen.

On claims with two E/M charges, such as this one, I always had trouble getting both paid on the first submission. I started appending modifier 25 to both E/M charges, now most get paid first time.

Make certain that your preventive / problem diagnoses are linked correctly.
