Can someone please help me with this procedure? I am thinking 64400 x 2?

DATE OF PROCEDURE: May 1, 2015
DIAGNOSIS: 729.2 - Neuralgia Neuritis & Radiculitis Unspec
INDICATIONS: Facial pain
ANESTHESIA: Local

DESCRIPTION OF PROCEDURE: After written informed consent was obtained, and risks and benefits discussed, including but not limited to, infection bone nerve spine damage. The benefits being pain relief and identification of a significant pain generator.
The patient was brought to the procedure room and placed Right Lateral decubitus on the procedure room table with a pillow under the head. Appropriate monitors were placed.

The Right cheek over the eye is prepped with Betadine. The Spraorbital and Infraorbital V2 nerve on the Right is blocked with a mixture of 0.25 mL of 0.25% Marcaine, 0.50 mL of 1% Lidocaine and 1 mL of 10mg/mL of Dexamethasone at each level.

After the injection the needle was removed and the puncture wound dressed.
The patient was removed to the recovery area in stable condition.

Fluoroscopic Time: None
PRE-OPERATIVE PAIN SCORE: 3/10 POST-OPERATIVE PAIN SCORE: 1/10
VITALS DURING PROCEDURE: O2 SAT: 95% Heart Rate: 71
