What would you recommend to code for the following procedure:
A patient has a CRT-D (biventricular or "multi-lead" defibrillator). The LV lead is dislodged and the generator needs to be replaced. I am going to:

1. change the generator
2. extract the existing LV lead
3. implant a new LV lead (usually 33225)

**Also, can we charge for the removal of the generator?

Any input is appreciated...thanks

No, the removal is part of the ICD exchange.
