Can anyone offer me suggestions on how this might be billed?

Rt femoral artery cut-down (Dr.A)
Exploratory lap with aortotomy and excision of suprarenal stent as well as proximal portion of the stent graft covering bilateral renals.
Aortorenal thromboendarterectomy and thromboembolectomy of bilat renal arteries and left acessory renal artery.
***This is a day after ELG repair*** 78 modifier
