Our physician removed dual chamber pacer with RV lead & Atrial Lead by transvenous extraction. He then inserted Dual chamber ICD with new ICD RV & Atrial lead. I am unable to locate anywhere in the CPT for removal of pacer and leads with upgrade to ICD and leads and I dont want to unbundle this procedure. Need help with codes would be greatly appreciated.
