Wiki Help please!!! - correct modifier

Recently Healthchoice has denied payment of A7003 and A7016 when billed with 94640 is anyone else experiencing this issue? They are stating in their denial to resubmit the claims with the correct modifier? I don't understand, can someone help me pleasE? Thanks

Some of our payers bundle A7003 with 94640, but most of them reimburse between 22% and 90% of the fee billed. I believe it is payer-specific.
