UHC denying cpt 64721 stating "A primary procedure code is required when the procedure code 64721 is billed."
Completed a reconsideration but they Upheld the denial. Anyone else having issues with UHC with this? We're researched UHC guidelines for hand & wrist surgeries but the ones we found do not apply to this cpt 64721. Any advise greatly appreciated.
