I need help choosing the right cpt codes for a surgery. My provider states on the op note he did a Laparoscopy w/fuluration of endometrial lesions and bilateral salpingectomy and endo biopsy and d/c?

I want to make sure I do no unbundle but I can not find any codes that inculde some of these together? Any help would be great:eek:
 
You can bill 58662, Laparoscopy with Fulgeration, 58661 Bilateral salpingectomy with modifer 52, and D&C (always sent for biopsy) 58120 with modifer 51
