Our medical software has given me a warning that in coding 99213 with 99173(vision test) I am unbundling. My question is..this patient came into our office for a follow up to encephalitis and while in the room with the dr, she decides she needs to ck her vision, as her vision could be affected as a result of encephalitis. If a vision test was not planned, but decided upon while the patient was here, could i not add modifier 25 to this as this E/M was not to include a vision test in the beginning?
