Any advise with a Nerve Root/Epidural injection?

Would this be coded as 77012/ 64490-50, 64491-50, 64492-50?

I have a Neuro MD that performed a Bilateral Transforaminal nerve root sleeve and epidural injections of contrast and fibrin glue on levels T7/8; T8,9; T9/10; and T10/11 under CT guidance. Would someone please help me with this coding? I know the contrast injection would not be coded due to this was just a confirmation of needle placement. But I do need some additional assistance with the rest. Any assistance would be much appreciated.:o
