Please help with coding for CPT 15002...doc want to use this code for the
Radical excision of scar tissue of back in preparation for
flap surgery, 5 cm2. Is this correct?


PREOPERATIVE DIAGNOSES: Severe atypical lesion of back,
possible melanoma, left side, scarring of left back status post
shave biopsy.
POSTOPERATIVE DIAGNOSES: Severe atypical lesion of back,
possible melanoma, left side, scarring of left back status post
shave biopsy.
PROCEDURES PERFORMED:
1 Wide excision of left back lesion, 3.1 cm.
2. Complex closure of back, 8.5 cm.
3. Radical excision of scar tissue of back in preparation for
flap surgery, 5 cm2.
4. Bilateral intercostal nerve blocks, six levels, for
postoperative pain management.

Next, she was taken to the operating room, padded appropriately,
and placed prone. Posterior back was prepped and draped in a
sterile fashion. The areas were infiltrated with 1% lidocaine
with 1:100,000 epinephrine; 9 mL delivered. Nerve block and
field infiltration was performed with 0.25% bupivacaine with
1:200,000 epinephrine; 6 mL delivered. Previously biopsy-prove
atypical possible melanoma was excised with 1-cm margins. It
was taken down to the fascial layersIt was sent to for pathologic
analysis. It was marked short superior and long left lateral.
The wounds were irrigated and hemostasis was obtained.
Tangential release was performed of all scar tissue; radical
excision in preparation for flap surgery; 5 cm2. Distinct
surgical procedure.
Large flaps were elevated. Undermining was performed equal to
the length of the incision, 8.5 cm. Flexor elevator
multilayered closure was performed with 3-0 Monocryl deep, 3-0
Monocryl deep dermal, 4-0 Monocryl deep dermal, 3-0 Monocryl as
running subcuticular stitch, and 5-0 Prolene interrupted in the
skin. Benzoin and Steri-Strips were applied. Medipore
dressings were applied. The patient was awakened and taken to
the recovery room in stable condition
