Wiki Help with diagnoses codes for pancytopenia due to myelodysplasitc syndrome

I find the 284.1 but it says excludes pancytopenia due to myelodysplastic syndrome (238.72-238.75) Does that mean that I can not use 284.1 at all or can I use 238.75 first followed by 284.1 or just 238.75 which does not address the whole statment?

Because 284.1 excludes the 238.75 then I would only code the MDS 238.75.
