Help... Can any one provide me with some documentation guidelines for performing cryotherapy in a Dermatologist's office. I am unable to find what needs to be documented and does a procedure note really need to be done.

documentaion for Cyro

We ask that our drs provide the number of locations as well as specifically where (example 10- neck 2- scalp 3-left arm) etc That has done us fine for years however when a provider has failed to detail out where the Cyro was done (and of course what for) and notes had to be submitted to insurance they would bounce it right back for no detail in document.
