Hello Coders, I code for a GI practice. I hope some one can help with this. When coding for an EMR 45390 and 43254 should the dictation from the doctor state that the EMR is cap-assisted or ligation assisted for removal of the lesion. Several of the doctors dictation include this information but several of them don't. I just want to get everybody on the same page. Thanks for any help.
