Note:
New patient presents today with nasal congestion, sinus congestion. Able to blow yellow discharge from nose. Congestion started yesterday. Pressure around his eyes. Took OTC meds that did not help.
Patient has no fevers, chills ear or throat pain, no cough. Patient does smoke ocasionally.
Vitals are WNL. NAD. HEENT: unremarkable. Skin: warm and dry. Lymphadenopathy nontender anterior chain. Heart: RRR, Lungs: clear to auscultation.
DX, Viral, no antibiotics required. 465.9
Pulse ox 98%

Please tell me if you level a 99203?!

New problem, 1 point for the pulse ox, and minimal risk.
