Wiki help with modifier 51 added to CPT 59400

I billed a 59400, global OB claim, to an insurance company and they added a modifier 51 to my claim then allowed a very small amount so they could pay out very little to the provider. Has anyone ever seen or heard of this happening before and any suggestions on how to appeal it? This insurance company says that they use CMS guidelines but I have never seen even CMS have such a low allowable amount for this global OB code. Please help! Thank you.
